INNOVATIONS PROGRAMS CONSULTANT

Temporary Part Time

For New Innovations Plan

Salary: $75-$100 per hour

Create Data Collection books, compile and evaluate gathered data. Create reports to be utilized for MHSA program development, implementation and improvement. Take a consulting role in assisting with the development of work products neessary for phases of MHSA Innovations Plans Implementaiotn.  Design, document, implement and manage data collection and reporting systems. Maintain up-to-date knowledge of MHSA regulations, policies and procedures as well as Agency Regulation, policies and procedures, disseminate knowledge to staff and ensure compliance on a continuous basis, maintain records and documentation of assigned program activities, ensure confidentiality and PHI for clients served by agency to comply with HIPAA regulations.

Ph.D. or Psy.D required in Psychology, clinical research or a related field.  3 years experience preferred in research and clinical settting.  Bilingual Spanish or Vietnamese preferred.

PSYCHIATRIC NURSE PRACTITIONER

for Adult Services

Salary:

$67,110- $104,536

Provide outpatient medication monitoring(supervised by psychiatrist) and support services to adults older than 18 years old with severe and persistent mental illness. Work collaboratiely with interdisciplinary clinical team to provide integrated, effective, and high quality behavioral health care to residents of Pomona, Claremont, and LaVerne with at least moderate-severe functional impairment due to mental illness.

MSN w/speciality in psychiatric/mental health nursing and completion of approved adult, child, or family practitioner program or completion of approved Mater's level psychiatric/mental health nursing program required.

Two years experience required in community mental health experience strongly preferred; computer skills required;aptitude in using electronic medical recotd; bilingual preferred in Spanish, Vietnanmese, Cantonese, Korean or Khmer. Prior experience with LA County DMH paperwork a plus.

COMMUNITY NAVIGATOR

Salary:

$15.89 - $20.65

Engage with people who need services and their families to help them quickly identify currently availabale services, including formal and informal supports and services tailored to the particular culture, ethnic, age, and gender identity of those seeking them. Recruit community based organizations, faith based organizations, and other community groups to become part of an active and ever growing locally based support network for people, including those most challenged by mental health issues. Follow up with people with whom they have engaged to ensure that they have connected with support structures and received the help they need. Use information technology and other means to map and keep up to date about the current availability of services and supports in the service area. Be a team player and have the ability to work in an environment that requires communication and engagement with other team members.

Personal or professional experience with mental health issues. Extensive experience working in or with commmunity and non-profit organizations, and working with people who need services and their families, as is an ability to work with and earn the trust of individuals and groups across different income levels.

Must possess a current valid California Driver License, a satisfactory drivign record, and a properly registered and insured vehicle.
