The Access to Care Department serves as the centralized point of entry for individuals and families seeking specialty mental health services through Tri-City Mental Health Authority (TCMHA). Access to Care facilitates screening, intake assessment, eligibility determination, care coordination, and linkage to appropriate behavioral health services for residents of Claremont, La Verne, and Pomona.

Individuals may access services by phone, walk-in, or referral. A behavioral health professional will speak with you about your needs, concerns, and goals for care to determine appropriate services and coordinate next steps, including scheduling an intake assessment when appropriate. If you’re eligible and agree to services, we will connect you to the program or services that is most suitable to meet your needs. If you are not eligible for services, our staff will provide referrals and warm handoffs to community providers and resources to help ensure continuity of care.
